Special Education - Transition Resources

Secondary transition is the process of preparing students for life after they leave high school, including participation in post-secondary education or training, employment, and community living. These three areas are often referred to as “post-secondary outcomes” and are the driving force behind Individualized Education Programs (IEPs) written for students in high school. Transition planning begins no later than age 14, in middle school or early high school, as students explore what they want their post-school outcomes to be through career awareness exploration activities. It continues through high school as instruction and community experiences support these outcomes. Devereux Center
Devereux is a special needs community of residential and day training and treatment center. It is a non-profit organization that enhances the life potential of persons 18 years of age and older. Devereux services individuals with intellectual and developmental disabilities, autism, emotional and behavioral disorders, and geriatric challenges. The Pocono Center is the closest facility to Wallenpaupack Area School District.

Goodwill Industries
Goodwill Industries offers many employment-related services, including job training.

Hiram G. Andrews Center
The Hiram G. Andrews Center is a post-secondary school located in Johnstown, PA. Hiram G. Andrews offers dormitories and accommodations for students with disabilities as they engage in meaningful learning opportunities and career training.

Human Resource Center (HRC)
The Human Resource Center, Inc. is a Not for Profit Corporation whose purpose is to afford support services to challenged adolescents and adults in vocational, community, and home settings.

JAN (Job Accommodation Network)
JAN provides free, confidential technical assistance about job accommodations and the American with Disabilities (ADA) Act.

PA Careerlink
Careerlink is a state agency that provides job placement assistance and a host of other employment related services. Pocono Counties Workforce Investment Area covers the following counties: Carbon, Monroe, Pike, Wayne.

PA Office of Vocational Rehabilitation Services (OVR)
OVR provides services for people with disabilities to help them become or stay employed. Available services include diagnostic services, vocational evaluation, vocational counseling, training, restoration services, placement assistance, assistive technology, and other support services. Planning for the Future Checklist
The Planning for the Future Checklist can be used as a tool to get ready for life after high school. The checklist is divided into three sections; Education/Training, Employment, and Community Living. Each section contains several transition activities that your child can do to help achieve their post-secondary transition goals and build on what they started the year before.
